NHS Medical Card (or NHS number from previous GP)
Passport (if applicable)
Asylum seeker card (if applicable)
Proof of name & address e.g. Utility bill (less than 3 months old), Council tax bill, driving licence, bank statement
Completed health questionnaire
Children under 16 cannot register without guardian/family
